Intercollegiate Athletics Pre-Participation Packet

Please refer to the PLU Health Center for information relating to intercollegiate athletics medical issues.

Returning NCAA Athletes: Once you have completed this online form (linked on the words Returning NCAA Athletes), you are done.  You will still have a physical in the Athletic Training Room prior to your first day of practice.  At this time you will be required to provide your insurance card to the Athletic Training Staff.

New NCAA Athletes: When you have completed this form (linked on the words New NCAA Athletes), you will receive an email.  You will then need to get a physical at the PLU Health Center prior to your first date of participation (your first practice). Please set that up with the Health Center as early as you can. When your physical is done, you will need to provide to the Athletic Training staff a copy of the front and back of your insurance card prior to your first practice.

New Athlete Preparticipation Medical Examination Information

All new athletes will be required to have their physical done on campus at the PLU Health Center.  This physical will be FREE of cost to you.  A detailed letter is below for more details on this new policy.  Please call the Health Center to schedule your physical.  Their summer hours are:

Monday-Thursday 9:00-5:00, closed for lunch 12:00-1:00
Friday 9:00-12:00
