Fill out the PLU Medical History Record(NEW LINK) and submit it to the Health Center through their secure server (pictured below) linked here: Etrieve Secure Document Upload.

Schedule a sports physical. We encourage athletes to complete a free physical with the Pacific Lutheran University Health Center. Contact information for the PLU Health Center is: 253-535-7337. Appointments for physicals are available during the summer and in-person registration (In-person registration days: June 13, 24, 27, 28, July 11, 15). THIS MUST BE SCHEDULED IN ADVANCE, walk-ins are not accepted.
OR If you are out of state, sports physicals can be done at home if it meets the following criteria:

They are performed by your primary care physician with knowledge of your medical history

Performed by a DO, RNP, or MD

Your Primary Care Physician MUST complete the PLU Pre-Participation Physical Exam Form (NEW LINK). Other exam forms will not be accepted.

Sports Physical must be completed between May 1st and August 1st
* If you decide to do a physical outside of PLU, ALL documents must be faxed to the PLU health center at 253-536-5042 AND schedule an appointment at the PLU Health Center to review your documents virtually with you. Please make sure that all your paperwork from the sports physical is signed properly by both the physician and yourself.

Print and fill out the NCAA Athletic Pre-Participation Packet BEFORE your sports physical. Bring this document to your sports physical for your health care provider to review and sign. Preview of the document is pictured below. IMPORTANT: Please make sure and read through the packet thoroughly.

Provide proof of sickle cell status. This is a new requirement as we no longer provide sickle cell waivers. Please check with your primary care physician if they have records of this. If not, please go to the following link (NEW LINK) for Information on how to get your sickle cell status from the state health department where you were born. If your state is not present you can use the CDC link to find the state associated with your birth.

Fill out the Athletic Training Online Medical Questionnaire. Preview of the medical packet is pictured below.

Schedule a virtual medical screen (preview of sheet is pictured below) with the Athletic Training Staff on the following google sheet document: Virtual Health Screen Sign-Up. *PLEASE USE PLU EMAIL.

Complete IMPACT baseline. Please look out for an email (pictured below) in the first week of June from “support@impacttest.com”. Please watch the instructional video linked within the email before you get started.

Primary insurance must meet the criteria outlined here: (Primary Insurance Qualification) Have your insurance card or a copy of your insurance card (Front & Back) available when your team arrives for medical clearance with the PLU Athletic Training Staff.

Additional written paperwork/clearance may be needed for but not limited to:
- Heart Murmur or other cardiac abnormalities
- Taking an NCAA banned substance (i.e. Adderall for ADD/ADHD)
- Absence of a paired organ
- Chronic illnesses
- Surgeries in the past 12 months
- Injuries that needed ongoing medical attention in the past 12 months
- Serious illness in the past year
- Confirmed or suspected diagnosis of COVID-19
