First Year and Transfer Checklist
#1: Fill out the PLU Medical History Record attached here: (PDF link) and submit it to the PLU Health Center through their secure server (pictured below) linked here: Etrieve Secure Document Upload. You will need to log into your PLU epass account in order to have access to the Etrieve secure website.
#2: Log onto your ARMS account. Log onto your ARMS account (link here) using your PLU email and password that you created. Find the section titled “Forms requiring immediate attention.” Click on the tab on the right hand side highlighted in orange titled “View all forms”. After clicking on the tab, please fill out and sign all of the forms required.
Please thoroughly review, fill out, and sign the following documents below:
- Emergency Contact and Insurance Form
- Concussion Fact Sheet
- Traumatic Brain Injury (TBI) Fact Sheet
- Sickle Cell Trait Fact Sheet
- HIPAA Privacy Practices and Consent Form
- Medical Examination and Authorization Waiver
#3: Print and fill out the NCAA Athletic Pre-Participation Packet attached here: (PDF link). IMPORTANT: Please make sure to thoroughly read the packet and fill out everything you know about your medical history.
#4: Schedule a sports physical. Sports physicals are HIGHLY ENCOURAGED to be done on your own and it needs to meet the following criteria:
- They are performed by your primary care physician (PCP) with knowledge of your medical history, or performed by a DO, RNP, or MD
- The NCAA Athletic Pre-Participation Packet is REQUIRED to be filled out prior, printed, and brought to your physical appointment.
- Your primary care physician or doctor who you saw MUST fill out and sign the PLU Pre-Participation Physical Exam Form attached here: (PDF link). Make sure to print the form and bring it to your physical appointment. OTHER EXAM FORMS WILL NOT BE ACCEPTED.
- Sports Physical MUST be completed between June 1st and August 1st
- Transfer Students will need to also complete a new physical with the PLU Pre-Participation Physical Exam Form regardless of prior physical exam.
Unable to schedule a sports physical. We will be conducting sports physicals on the first day you report for training. The sports physicals will be conducted by our sports medicine team physicians. The NCAA Athletic Pre-Participation Packet is REQUIRED to be filled out, printed, and brought with you prior. Please contact the Assistant Athletic Trainer Caylie Shiramizu at shiramca@plu.edu if you need to schedule a sports physical appointment with us or if you have any further questions.
#6: Upload both the NCAA Athletic Pre-Participation Packet and PLU Pre-Participation Physical Exam Forms to your ARMS account. Log onto your ARMS account (link here) using your PLU email and password that you created. Find the section titled “Forms requiring immediate attention.” Click on the tab on the right hand side highlighted in orange titled “View all forms”. Upload both forms under their appropriate titles and sections. Sign and date attesting that you have properly completed both forms.
#7: Provide proof of sickle cell status. This is REQUIRED by the NCAA as we no longer provide sickle cell waivers. Please check with your primary care physician if they have records of this. If not, please go to the following website link attached here: (link to website) for information on how to get your sickle cell status from the state department where you were born. If your state is not present you can use the CDC link to find the state associated with your birth.
*If you’re unable to obtain your sickle cell documents from your primary care physician or the website provided, the Pacific Lutheran University Health Center will be able to provide a sickle cell test for you. Call them at 253-535-7337 to schedule an appointment. They will charge $25.00 for it. They will make sure to notify us when your sickle cell results have been completed.
#8: Schedule a virtual medical screen. (preview of excel sheet is pictured below) with the Athletic Training Staff on the following google sheet attached here: Virtual Health Screen Sign-Up. *PLEASE USE PLU EMAIL WHEN SIGNING UP.
*The emergency contact and insurance form, NCAA athletic pre-participation packet, and the PLU pre-participation physical exam forms are REQUIRED to be completed and uploaded prior to scheduling your virtual medical screen appointment with the Athletic Training Staff. If those items are not completed and uploaded, YOU WILL NOT be allowed to have your virtual medical screen appointment.
#9: Complete IMPACT baseline. Please look out for an email (pictured below) in the first week of June from “AtHomeTest@impacttest.com.” Please watch the instructional video linked within the email before you get started.
#10: Primary insurance must meet the criteria outlined here: (link here). You must upload a photo of the front and back of your insurance card onto ARMS under the emergency contact and insurance form prior to reporting to campus.
#11: Additional written paperwork/clearance may be needed for but not limited to:
- Heart murmur or other cardiac abnormalities
- Taking an NCAA banned substance (i.e. Adderall for ADD/ADHD)
- Absence of a paired organ
- Chronic illness
- Surgeries in the past 12 months
- Injuries that needed ongoing medical attention in the past 12 months
- Serious illness in the past year
- Confirmed or suspected diagnosis of COVID-19
