Pre-Participation Physical Exam Form (PDF)
  • FIRST YEARS and TRANSFERS only
  • This form must be filled out by your primary care provider if doing a physical outside of PLU
  • Only physical form accepted by PLU

ADHD Medical Exception Form
  • Along with the exception form, the athletic training staff should have documentation, that can be a letter or copies of medical notes, from the prescribing physician that documents how the diagnosis was reached, and that the student-athlete has a medical history demonstrating the need for treatment with the banned medication. The letter should contain information as to the diagnosis (including appropriate verification of the diagnosis), medical history and dosage information.
